Sunderland Care and Support provides 'Independence at Home' services across the city.
These services provide person-centred support and planned care for people with learning disabilities and mental health needs. They help people to live in the local community, in their own tenancies, by supporting their independence in line with their assessed needs.
This includes a team of Support Time and Recovery workers who provide mental health services working across boundaries of care, organisation and role.
Sunderland Shared Lives
The Sunderland Shared Lives Service opportunes adults with learning disabilities:
- To be looked after in the homes of carefully chosen carers
- To live in a family environment
- To have a full and independent life where they can be part of their local community
Carers are supported by Adult Family Placement Officers and are provided with regular support and training.
