Practical Advice and Support Service

PASS

The Sunderland Care and Support Practical Advice and Support Service (PASS) provides person-centred, outcome-focused support to adults aged 18 and over who live in their own homes within the City of Sunderland and have been assessed as eligible for care and support under the Care Act 2014. The service supports individuals whose needs arise from mental health conditions and/or cognitive impairment, as well as learning disabilities, including autism and behaviours that may challenge.

PASS is built on an independence-focused and strengths-based approach, working alongside individuals to help them develop, regain, or maintain the skills needed for everyday living. The service provides time-limited, goal-oriented support that is shaped around what matters most to each individual. This includes building confidence, promoting wellbeing, and enabling people to maintain their independence within their home and community.

Staff work in partnership with individuals, encouraging learning, self-management, and active participation, rather than undertaking tasks on their behalf. The approach recognises and builds upon each person's existing abilities, focusing on what individuals can do and supporting them to achieve their personal goals. Support is tailored to individual needs, preferences, and identified risks, ensuring a flexible approach that promotes independence while maintaining safety and wellbeing.

All individuals referred to PASS will have received a comprehensive assessment completed by Adult Social Care (ASC). Once a referral is received, support planning is co-produced, ensuring individuals are actively involved in setting their goals and shaping how their support is delivered.

The overall aim of PASS is to empower individuals to achieve greater independence, develop sustainable skills for everyday living, and maintain their wellbeing within their own home and community. By focusing on strengths, promoting choice and control, and delivering tailored support, PASS enables individuals to live as independently and confidently as possible.
