Supported Living

Sunderland Care and Support's Supported Living Service, supports adults to live independently in their own homes and communities. Our Supported Living services are registered with the Care Quality Commission (CQC) and are delivered in line with regulatory standards that promote safety, wellbeing, and high‑quality care and support.

We provide flexible, housing‑based support for people with mental health needs, autism, learning disabilities, and physical or complex care needs. Supported Living is designed for people who want to maintain choice and control over how they live, with support tailored to their individual strengths, preferences, and goals.

The people we support live in their own self‑contained accommodation, such as a flat or house, either alone or shared, depending on their wishes and assessed needs. Support can range from a small number of hours each day to 24‑hour support, including overnight support where required.

Co‑production is at the heart of everything we do. We work alongside the people we support as equal partners, recognising them as experts in their own lives. Support is shaped through ongoing conversations, shared decision‑making, and collaboration to ensure services reflect what matters most to each individual.

Our Supported Living Services are delivered through four specialist divisions:

  • Mental Health
  • Autism
  • Learning Disability
  • Physical and Complex Care

Each division provides tailored support while sharing a consistent approach focused on independence, dignity, safety, and quality of life.

We work closely with individuals, families, Adult Social Care, and health and community partners to ensure support is coordinated, responsive, and centred on achieving meaningful outcomes.
